Social Prescribing &
Community Connector Frailty
Kilbryde Hospice employs a Social Prescribing Link Worker. To find out more about the role of a Social Prescribing Link Worker please click here.
You can be referred to the Social Prescribing Link Worker if you live in the East Kilbride / Strathaven area and are living with a life-limiting diagnosis e.g. cancer, COPD, progressive neurological diseases, end-stage heart disease and frailty.
Instead of solely relying on traditional medical treatments, the Social Prescribing Link Worker will meet you and together decide on what your requirements are. This may be an introduction to a social activity, an exercise class, or engaging in a local community group. This is particularly beneficial for people experiencing loneliness, chronic health conditions, or mental health issues.
