Attending the hospice from your home.

Attending the outpatient clinic

Attending an Outpatient Service at the hospice building is often the first opportunity for people to get to know the hospice team. Access to our Outpatient Service is by referral only however if this is proving difficult please call our Clinical Admin team on 01355 202020 Option 3 to discuss.

Can people be accompanied?

Yes, carers and relatives are very welcome to come along to all outpatient appointments. It can be useful for the team to be aware of family members’ concerns.

For referral to Outpatient Services please ask your GP or specialist nurse to refer you using the following form below. If you experience any delays you can also self-refer for these services.
