How to make a referral to our Clinical Services

Read our information below on how to refer a patient to Kilbryde Hospice

Non-In-Patient Unit referrals ( CLAN, Day Services, Carers, Wellbeing and Sunflower Service)

We operate a healthcare professional referral line for Non-Inpatient referrals which operates between 10:00hrs – 15:00hrs, excluding weekends and public holidays. Please contact Clinical admin on 01355 202020, option 3 and ensure that you have access to the patient’s record.

Advice regarding this service is available during office hours from the Clinical Admin on 01355 202020, option 3. Completed referral forms should be submitted whenever possible by secure email (eg.@any.scot.nhs.uk) to the designated hospice referral email address.

In-Patient Unit referrals

For all referrals to Kilbryde Hospice Inpatient Unit please call Specialist Palliative Care team Hub on 01698 755141 from Monday to Friday 0900-1700. Please have all the referral details available as the team will triage your call and mutually decide which of the hospice services is best suited to the patient in your care. Please note the hospice is not a long-term place of care.

 

  • Please consider the reason for referral (symptom control/rehabilitation/ or end of life care)
  • Patient’s awareness of referral, diagnosis, and consent
  • Prognosis (days, weeks, months, don’t know)
  • Main symptom(s) with an indicator of severity (mild, moderate, or severe)
  • Main contact details
  • Information regarding the patient’s current care environment- please note Kilbryde Hospice does not offer long term care. If a patient stabilises then we will discuss discharge home or to a homely environment.
